Enrollment dates for elementary students continue through Friday, Aug. 9, 2013, from 9 a.m. until 7 p.m. at the following locations:
• Ada Early Childhood Center, pre-kindergarten and kindergarten, 630 W. 33rd, 580-310-8283.
• Washington Grade Center, third and fourth grade, 600 S. Oak, 580-310-7303.
• Hayes Grade Center, first and second grades, 600 S. Mississippi, 580-310-7294.
•Willard Grade Center, fifth and sixth grades, 817 E. 9th, 580-310-7250.
To shorten the enrollment process, parents should take advantage of enrollment forms available to download and complete at your convenience to bring with you on enrollment day. For additional enrollment information and enrollment forms, visit Ada City Schools website at www.adacougars.net.
All students enrolling in the Ada City School District for the 2013-2014 school year must bring the following to enrollment: immunization record, birth certificate, Social Security card, Medicaid number, and CDIB (Certificate of Degree of Indian Blood) card, proof of residency. Immunizations must be complete before any student may attend school.
Enrollment at all Byng School sites: Byng High School, Byng Junior High, Byng Elementary, Francis and Homer, continues Aug. 7-8.
The office will be open from 9 a.m. until 3 p.m. Aug 7 and from 11 a.m. until 6 p.m. Aug. 8.
New students will need proof of residence, Social Security numbers for each student, immunization records, birth certificate and CDIB card (if appropriate.)
New students who are residents of Latta district (pre-kindergarten through fifth grade) will enroll Aug. 7 from 9 a.m. until 3 p.m. at the elementary office. New students must take proof of residency in the district, as well as the name and address of last school attended.
All students who attended Latta Elementary (pre-kindergarten through fifth grade) last school year will enroll Aug. 8 from noon until 6 p.m. at the elementary office. All students who have had new immunizations should take updated immunizations record with them to enrollment.
Latta pre-kindergarten and kindergarten students who have not attended a public school must take birth certificate, immunization records, CDIB card if applicable and proof of residency if residing within the district. A readiness screening and /or a speech and hearing screening may also be schduled at the time of enrollment. Please be prepared to make time for these screenings.
